CWS Community Service Travel Grant and Quick Fix Grant Programs Contracts
In order to receive your grant check, you must complete and/or gather all of the contrcts and paperwork below and submit them to the CWS.
- Parents/guardians may fax their completed form to Melissa Hawkins at 781-283-3674.
Students and parent/guardians signatures may appear on separate forms.
Required from all students:
- Community Service Travel Grant and Quick-Fix Grant Contract - acknowledges the receipt of funds from the CWS and the requirements of the program
- A copy of your medical insurance card
Additional forms required for students completing domestic community service projects:
- Assumption of Risk and Release - this contract must be signed by both the student and her parent/guardian
- Domestic Student Worksheet for Emergency Contact Information
Additional forms required for students completing international community service projects:
- Assumption of Risk and Release - this contract must be signed by both the student and her parent/guardian
- International Student Worksheet for Emergency Contact Information
- Parent Safety Letter
- A copy of your passport, if applicable
- You must register with the U.S. Embassy or Consulate in the country where you will be staying. Register on-line at: http://travel.state.gov/travel/tips/registration/registration_1186.html Once you have registered, please print out the registration page submit a copy to the CWS.
