Transfer and Exchange Students Wellesley welcomes each semester students who transfer from various schools as well as students participating in exchange programs. Listed below are the pre-registration forms and other information that will help you prepare for your arrival at Wellesley. Required Material: Health Forms. Please print these forms out, take them to your physcian to complete, and mail to Wellesley College Health Services, 106 Central Street, Wellesley, MA 02481 or fax the forms to 781-283-3693. Medical Insurance Waiver Form This is an electronic form, which you reach by choosing the student information, and then using the drop-down menu to choose Wellesley College, and then following the information for spring arrivals. . Room Assignment Form This a word document. Don't be concerned about the due date, since this is the same form we use for students who arrived last fall. You will take the Quantitative Reasoning Assessment when you arrive and this booklet will help you prepare for that.
