apply for Life Insurance?
Steps you need to take:
- Contact the Benefits Office for enrollment forms.
- Sign up for your group term life insurance and
designate a beneficiary within first 30 days of employment.
- Contributory Life Insurance is also available during
this time. You may elect to participate
in this plan and receive additional insurance.

All regular full-time
employees are eligible for the group term life insurance coverage. You are
eligible to be enrolled on the first of the month coincident with or next
following your employment date.
