spouse has an employment change?

Steps you need to take: 

Notify the Benefits Coordinator X2231 or X2212 immediately.  You only need to do this if your spouse was carrying the health/dental insurance, and now you need to change to the College’s health/dental insurance if you wish to carry family plan.  You only have 30 days from date of qualifying event (termination of coverage) to change to the College’s health/dental plans.

Within 30 days of the date of termination of coverage, you must present the Benefits Coordinator with proof of termination of coverage from your spouse’s employer.

Other things to consider:

When enrolling a spouse and/or dependents in health/dental coverage, you will need to provide copies of your certified marriage certificate and/or birth certificates, as appropriate.
