We are located in Simpson, between Bates/Freeman Halls and the Science Center. You can access our entrance by taking the uphill path from the Science Center.

If you receive treatment outside of Health Services that you would like to share with us and be part of your medical record here (vaccines, hospitalizations, etc.), please complete a Release of Information and we can request the information from the provider.

To obtain copies of your immunizations or medical record
please fill out the attached release of information
and fax it to us at 781-283-3693.
There will be a fee of $10.00 for immunizations and
$15.00 for the complete medical record.
Your request will be processed within 5-7 business days.
