Faculty may use this form to submit information for all materials needed to be placed on course reserve. Ten items can be entered on each form, and you may submit as many forms as needed to list all of your materials.
To be sure our information is correct and up to date, please submit a new list each time you are offering a course. If you would like assistance identifying material you have used in the past, please note this in the Special Instructions box below.
Please list all materials: personal copies, books, videos, etc. Complete bibliographic information for each item, must be provided by the faculty member. If a Library copy is not available and cannot be purchased in time, faculty may bring personal copies of books or videos to the Knapp Center Service Desk.

Please submit lists for Fall 2008 by June 13th in order to be sure that material is ready for the first day of classes. Lists will be processed in the order they are received.
Required videos submitted prior to June 13th will be offered as video e-reserves. Required videos submitted after June 13th will be digitized in the order they are requested. Videos that cannot be offered via video e-reserves will be placed on reserve in Clapp. Recommended videos will be held in the regular collection but will be restricted to use by the Wellesley Community.
