Science Reserves Web Form

Faculty may use this form to submit information for Library materials needing to be placed on course reserve. Four items can be entered on each form. Submit as many forms as needed to list all of your materials: books, videos, etc. Complete information for each item must be provided.

Articles or personal copies should be brought and logged in room L002 in the Science Library.
