Study Abroad Forms and Handbooks For Accepted Students
PRE-DEPARTURE HANDBOOK
Students who have been accepted to a study abroad program should review the Study Abroad Handbook very carefully. This book will be an essential reference before and during the semester or year away.
Leave Of Absence Form Once students are accepted to a study abroad program, they need to complete a Leave of Abscence form. This should be submitted electronically no later than April 1 for fall semester study abroad and November 1 for spring semester study abroad.
Study Abroad Contract and Waiver
All students studying abroad and their parents are required to complete the Approved Study Abroad Participation Agreement & Assumption of Risk and Release. Signed forms are due in the OIS by May 1 for fall semester study abroad and December 1 for spring semester study abroad. They can be returned in person, by hand or by fax (781-283-3618). No tuition monies will be paid to the program provider on behalf of the student until these signed forms are submitted.
Health Form for International Travel
All students planning to study abroad are required to complete a health information form for Wellesley College Health Services by April 1 for fall semester/full year study abroad and by November 1 for spring semester and wintersession study abroad. In addition, students are strongly encouraged to take advantage of pre-departure services, including advising on destination-specific health issues and immunization requirements. (Students with pre-exisiting conditions, relating to physical or mental health, will need to be seen by their practitioner during the semester preceeding study abroad.)
Housing Preferences
Prior to departure for study abroad, all students should submit a Housing Preference Form to the Housing Office to ensure that they receive housing on campus upon their return to Wellesley.
Course
Confirmation Form
In order to ensure that your study abroad courses transfer back to Wellesley, please submit this form as soon as you have made your initial course selections. The Director of International Study will review and approve the courses for general degree requirements. Any courses that are intended to fulfill major, minor or distribution requirements will also need the approval of the relevant department chair (using the Transfer of Credit Form, below). Please refer to your Study Abroad Handbook for information on courses that may not be eligible for Wellesley credit.
Transfer of Credit Form
Students who wish to use study abroad courses to fulfill major, minor or distribution credit need to fill out a Transfer of Credit form for each course and each requirement. Students complete part one of the form and submit this to the OIS along with a course description. Please refer to the detailed instructions for more information.
