Listed below are forms that students may need to fill out at certain points of their Wellesley career.

Please note that some forms are not currently available online.  Please consult your Class Dean with any questions. 

• Acceleration Petition.  This form opens in a WORD document. Please fill it out, obtain the required departmental signature and then bring it to your Class Dean.
