Confidentiality/Release of Medical Information
- Please be assured that all medical information, records and data will be kept in strictest confidence within the athletic training staff. The athletic training staff may include: Team Physician, Health Center staff, athletic training aides, and other pertinent medical personnel.
- The athletic training staff and Team Physician are also authorized to disclose information regarding any injuries that you might receive during the course of the season, as well as your general fitness to play, to coaches or departmental administrators as needed.
- You have the right to withdraw this consent at any time, which is to be done in writing. However, participation is contingent on the ability to release and share information with the above mentioned parties.
