Forms

The following request forms are presented in PDF and Word Document format. You may print either form and fill it out and mail or hand deliver to Disability Services, 316 Clapp Library, 106 Central Street, Wellesley, MA 02481 or e-mail to the Director of Disability Services at jwice@wellesley.edu. The Disability Disclosure form can be used for any request (students, staff, faculty and guests), though the other forms are useful in ensuring that the correct information has been provided. Note that the ACCESS Van and Meal Plan Accommodation Forms are for students.
