Senior Checklist

Wellesley College Health Service Senior Checklist  

[ ] Locate a new doctor, primary care and /or GYN.  It may take up to 6 months to get an appointment!

[ ] If you are over 21 and have not had a PAP test, consider making a GYN appointment at Health Service before you graduate (x2810).

[ ] College Health Insurance expires on August 14, 2013. If you are staying in Massachusetts, make sure to review all of the insurance coverage options at http://www.mahealthconnector.org for more information.

[ ]  Contact University Health Plans if you have student health insurance questions www.universityhealthplans.com  or 800-437-6448.

[ ] Prescriptions will no longer be filled by the Health Service or the Counseling Service after graduation (5/31/13).  Be sure that you have a 3 month supply prior to leaving!

[ ] Speak with a nurse (x2810) to find out if you are up to date on all vaccines and to receive any that you may need.

[ ] Obtain a FREE copy of your complete immunization record by 5/24/13.  This will be required for entry into graduate schools and by some employers.  A release of information is available on our website www.wellesley.edu/healthservice.  Post-graduation fees for medical records are: $15 Complete Record;  $10 Immunizations

[ ] Return all crutches and canes by 5/24/13.

[ ] All payments are due by 5/24/13 or will be transferred to your student account.

[ ] Update all addresses/ phone numbers in Banner.

Health Service will be open on an abbreviated schedule from Monday, May 20 through Thursday, May 30

Clinical Hours will be from 10am-2pm for URGENT CARE ONLY
