Once on the site, employees will have the ability to file STD & EOI
claims/application and/or check the status of their application. The
following are quick outlines of the STD & EOI Resources available on the
site:

- Click on "Apply for Evidence of Insurability on-line"

      Our secure on-line system allows employees to provide all the
      information needed for Evidence of Insurability in about 10 to 15
      minutes. Following completion of the application, the employee
      receives confirmation by e-mail. The employee will then receive
      notification of our decision by e-mail or mail.

Open Enrollment 11/5-11/16

No medical evidence for employees electing the lesser of 2x salary or $300K. No medical evidence for spouses electing $15K (employee must be enrolled in contributory coverage).
