Please fill out the appropriate form for the equipment you need moved:
If you need multiple phones and computers moved, please use this Google Spreadsheet template. After opening the spreadsheet go to File > Make a copy. Once you have saved a copy to your Drive, please share the spreadsheet with email@example.com, firstname.lastname@example.org and email@example.com. The spreadsheet includes an Instructions sheet with information on how to fill it out. Once the spreadsheet is complete, LTS staff will contact you to review the information provided.
If you require a new computer, printer, or software, please fill out the Authorization to Purchase form.
