Reporting of Accidents & Injuries on Campus

Employee accidents and injuries must be reported via the Human Resources (HR) Accident Report & Treatment Form (ART form) within 24 hours of the incident, unless the accident resulted in a work-related fatality in which case it needs to be reported within 8 hours. This form is filed with both Wellesley College’s Human Resources and Environmental Health and Safety offices.


Please note: student employees of the College, similar to faculty and staff employees, should also report all job related accidents and injuries on the ART Form.
