The Academic Accommodation Request is for students with documented disabilities who wish to request testing or classroom accommodations.
Course Accommodation Request for previously approved accommodations
Once approved for academic accommodations, this form must be submitted each semester to request accommodations needed for current courses.
The ACCESS Van Request is for students with mobility difficulties requesting transportation around campus.
ACCESS Van & Rides Service Request for previously approved accommodations
Once approved for transportation accommodations, this form must be submitted each semester to request services.
ACCESS Van Off-Campus Transportation Request for off-campus medical appointments
Rides to off-campus medical appointments will be provided Monday through Friday 9:00 a.m.-3:30 p.m., at times when the ACCESS Van is not needed on campus. See schedule of most likely ride times.
The Disability Disclosure form may be used for any request not covered by other forms on this page. It is intended for use by students, staff, faculty and guests.
Housing Accommodation Request Form and Healthcare Documentation Form for NEW Requests
This form is for students who have a documented need for a housing accommodation and are making a new request. The deadlines for submitting a housing request are as follows:
- Fall Semester – March 1
- Winter Session – December 1
- Spring Semester – November 15
- Summer Session – May 1
- Incoming First Year students – June 11
Requests submitted after the deadlines above, will be reviewed within a month of the request, and without the benefit of the room options available prior to the housing lottery.
This form is for students who have previously received a housing accommodation. Students do not automatically receive accommodations for subsequent semesters, and must submit this form each year by March 1 for the Fall semester, and by November 15 for the Spring semester. Requests submitted after the deadlines above, will be reviewed within a month of the request, and without the benefit of the room options available prior to the housing lottery.
The Meal Plan Accommodation Request is for students whose dietary needs can not be met by Wellesley Fresh dining options.
The Parking Request form is for use by students, staff, faculty and guests with permanent or temporary disabilites.
Service/Assistance Animal Registration for previously approved requests
Once your request for a service/assistance animal on campus has been approved, you must register your animal using this form each semester.
